Millions of women suffer from cellulite, a dimpling of the skin caused by uneven fat cells under the skin and seen most often around the thighs and buttocks. Even thin women can have cellulite and many have struggled for years to rid themselves of the 'cottage cheese' appearance. New treatments are available to reduce the appearance of cellulite and restore confidence in smooth, healthy skin.

"We know that cellulite involves multiple layers of the skin, including fat cells that are in an abnormal location in the dermis as well as an irregular arrangement down in the deeper fat layer of the tissue," says renowned New York dermatologist Dr. Neil Sadick. "So, by having a newer technology like Reaction by Viora that can target both mid-dermal as well as subcutaneous fat cell targets - and also be able to heat the surrounding septum around fat cells - we are able to produce a more effective cellulite outcome for our patients."

Reaction by Viora differs from other cellulite reduction treatments because it is fully adjustable. "Well, what's really new and exciting about it is that there are multiple frequencies that can target both more superficial tissue as well as deeper tissue. So you can have high intensity, deeper penetrating, again, you can have higher intensity superficial frequencies that target the dermis and you can also have more volumetric or deep bulk heating of deeper tissue in the fat layer. So the ability to target all of the different layers with differential radio frequencies is one of the major advances in terms of this technology," says Dr. Sadick.

He also says that this treatment is really appropriate for all saying that it can be used by "any woman that wants improvement of her cellulite and wants to move to the next level with a more advanced technology.... You can improve all patients with all clinical classification types of cellulite. Certainly those who have the more milder/moderate classifications of cellulite will do best but even those that have more significant cellulite or grade 3 or 4 can benefit from this type of treatment modality."

Multiple treatments are needed to see the final result. As Dr. Sadick describes, "Usually we recommend about 3-5 treatment sessions to see optimization. We are presently beginning a study looking at the optimal treatment parameters and again the optimal number of treatments that can be utilized to get the most efficacious results. In addition to that we know that cellulite is an ongoing issue so there is some degree of maintenance program that's necessary as well. Our research group in New York is also looking at new again pathophysiologic mechanisms of cellulite how the fat cells are different in terms of blood flow, lymphatic drainage and other metabolic parameters in the fat cells associated with cellulite so we're looking at new ways to really look at the science of cellulite and better target such as Viora that will give more efficacious outcomes as far as treatment."

Patients are able to see results relatively soon and regular maintenance is simple. "I think most people see significant results after two or three treatment sessions and most individuals have continued improvement for approximately 6 months and they will require a single maintenance treatment to continue the outcome." He says that patients should return for follow up treatment every six months.

Dr. Sadick says that Reaction can be used in conjunction with other methods to improve results. "Well if you have cellulite by itself you can use it as the sole modality there are new again, mesotherapy or injectable lipolysis type of treatments that are now being researched by companies such as Ulthera and Kythera as well. We are working on a new scientific injectable product also that can induce lipolysis so I think at some point you will see combination treatments including injectable fat dissolution again in conjunction with these types of radio frequency heating sources that again remold the fat cell as well as tighten the connective tissues - stroma around the fat cell - and then push the fat cells back down to their normal subcutaneous compartment where they belong. Combination therapy will probably in the future be utilized to give optimized outcomes."

The number of noninvasive cosmetic procedures rose 47% between 2008 and 2009, according to new survey results released by the American Academy of Facial Plastic and Reconstructive Surgery (AAFPRS). The results show a continuation of the trend from the previous year, where consumers are opting to have less invasive procedures. The economy is playing a large role in the growing trend, says the survey, which was released last week.

Not only are consumers having less-costly procedures, but 80% of physicians surveyed say patients are making these choices in order to remain competitive in the workplace.

The increase in noninvasive procedures administered by facial plastic surgeons was seen across the spectrum of treatments, with the greatest increase being poly-l-lactic acid (up 71%). Other increases include chemical peels (up 52%), hyaluronic acid (up 48%) and Botox (up 45%).

The survey also showed that 77% of physicians agree that patients are more educated about plastic surgery options. According to the survey, patients are researching different physicians and treatments to find the best one suited to meet their personal needs. Awareness of various procedures, appropriate physicians, best results, and recovery times play an important role in the decision-making process.

In 2000, facial plastic surgeons reported the face lift as the most common cosmetic surgical procedure, followed by blepharoplasty, rhinoplasty,  and ablative skin resurfacing. These procedures are most commonly performed on patients between the ages of 35 and 60 years of age. For female patients under age 35, the most common procedure performed was rhinoplasty.

The surgical procedures with the largest increases during the past year were ablative skin resurfacing (57%), revision surgery (18%) and face lifts (14%). Decreases were seen in lip augmentation (52%) and rhinoplasty (13%).

Physicians continue to express concern with procedures performed at medical spas. Seventy-six percent of facial plastic surgeons surveyed say they are aware that some medical directors of medical spas are not actually on site for medical treatments.

Most cosmetic procedures are performed in a private office (54%) setting. Procedures performed in a hospital came in at 23%, and the number of procedures performed at medical spas remained unchanged (2%). Half of the physicians surveyed are aware of misleading advertising involving injectables, lasers and other cosmetic therapies, and 35% of physicians believe medical spas will become a more popular location for patients undergoing treatments.

Breast Augmentation – down 6% over 2008
Nose Reshaping – down 8% over 2008
Eyelid Surgery – down 8% over 2008

Tummy Tuck – down 5% over 2008

 

While all of the cosmetic surgical procedures are down, the only top 5 minimally invasive procedure that dropped in 2009 was BOTOX.

A new study published by the Journal of Cosmetic Dermatology reports that using Botox may actually cause additional wrinkles. But before Botox fans start working on their worry lines, it's important to understand specifically what the study notes, and what it doesn't.botox.jpg

David Becker, MD, an assistant professor of dermatology at Weill Cornell Medical College in New York, says that when a patient uses Botox to immobilize certain areas of the face, other facial muscles will remain active when the patient tries to express emotion. Since any repeated muscle movement can cause wrinkles over time, the study is recognizing that the face will still move even with Botox in certain areas, and those movements can cause wrinkles.

"Paralysis of a set of muscles," he suggests, "might lead to recruitment of other muscle groups in an attempt to reproduce the conditioned activity being blocked, resulting in more prominent muscle activity in adjacent regions."

Additionally, it is noted that the movement of muscles untreated by Botox may result in more "nontraditional" wrinkles. Rather than "crow's feet" wrinkles around the eyes, a "bunny line," or horizontal line across the bridge of the nose, may appear.
